Legacy Health and Legacy Medical Group.
August 7, 3:00-6:00pm (SHS Locker Rooms).
Cost is $20, which will be donated back to the district.
Please bring completed sports physical form
(https://osaa.org/parents-students) or pick one up at the athletic office.
Male and female providers available.
